This General Consultation Agreement ("Agreement") is entered into as of the date signed below between the Law Office of Carl G. Hawkins, PLLC ("Firm"), and the undersigned individual or entity ("Client").
1.1. The Firm agrees to provide Client with a single initial consultation of up to fifty (50) minutes on the legal matter(s) identified by Client at the time of scheduling. The consultation is informational in nature. Booking a consultation does not create an attorney-client relationship. No legal representation, filing, or formal legal advice specific to Client's individual circumstances is guaranteed unless a separate written engagement agreement is executed by both parties.
2.1. The consultation fee is $250.00 for a 50-minute session, charged at the time of booking. Payment is due prior to the consultation. The Firm reserves the right to decline to proceed if payment is not received.

3.2. Consultations are scheduled through the Firm's online booking system. The cancellation and rescheduling policy set forth in the booking confirmation is incorporated herein by reference.
4.1. Booking a consultation, submitting a contact form, or making payment through this website does not, by itself, create an attorney-client relationship. An attorney-client relationship is established only upon execution of a written engagement agreement signed by both Client and the Firm. Until such an agreement is executed, the Firm has no obligation to maintain the confidentiality of information submitted through any intake or contact form.
5.1. All information shared by Client during the consultation will be treated as confidential to the extent permitted by the Florida Rules of Professional Conduct. The Firm will not disclose Client's confidential information to third parties without Client's consent, except as required by law or court order.
6.1. Nothing in this Agreement or in any consultation constitutes a guarantee, warranty, or prediction of the outcome of any legal matter. The Firm provides legal information and general guidance based on the facts presented during the consultation.
7.1. The Firm will conduct a reasonable conflicts check prior to the consultation. If a conflict of interest is identified, the Firm will notify Client and may decline to proceed. Client agrees to disclose all parties involved in the matter at the time of scheduling.
8.1. Consultation fees are processed through a third-party payment processor. By submitting payment, Client authorizes the Firm to charge the payment method provided for the applicable fee. Client's full card number is stored securely by the payment processor, not by the Firm directly.
9.1. This Agreement shall be governed by and construed in accordance with the laws of the State of Florida. Any dispute arising under this Agreement shall be resolved in the state or federal courts of Duval County or St. Johns County, Florida.
10.1. This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior discussions, representations, and agreements.
By signing below, Client acknowledges that Client has read, understands, and agrees to the terms of this General Consultation Agreement, including the cancellation and no-show policy in Section 3.
